CORONAVIRUS/COVID-19 WAIVER AND RELEASE 

The novel coronavirus/COVID-19, has been declared a worldwide pandemic by the World Health Organization. Coronavirus/COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. By signing this Waiver of Liability, you are representing to Ballistic Budo Club, Inc (BBC) the following: 

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and other public health authorities still recommend practicing social distancing. 

I acknowledge that BBC cannot guarantee that either myself and/or my child/children will not become infected with the Coronavirus/COVID-19. I understand the risk of becoming exposed to and/or infected by the Coronavirus/COVID19 may result from the actions or omissions of myself and others, including but not limited to, BBC staff, other clients and their families. 

I voluntarily seek services by BBC and acknowledge that by doing so, I am increasing my risk and/or my child/children risk to exposure to the Coronavirus/COVID-19. 

I attest that I, and/or my child/children: 

1. Are not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell; 

2. Have not traveled internationally within the last 14 days; 

3. Have not traveled to any highly impacted area within the United States in the last 14 days; 

4. Have not been exposed to someone with a suspected and/or confirmed case of Coronavirus/ COVID-19; 

5. Have not been diagnosed with Coronavirus/COVID19 and not yet cleared as non-contagious by state or local public health authorities; and 

6. Are following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID19. 

I hereby release and agree to hold BBC harmless from, and waive on behalf of myself, my child/children, my heirs and any personal representatives any and all causes of actions, claims, demands, damages, costs, expenses and compensation for damage or loss that may be caused by any act or failure to act of BBC, or that may otherwise arise in any way in connection with any services received from BBC. 

I understand that this release discharges BBC from any liability or claim that I, my child/children, my heirs, or any personal representatives may have against BBC with respect to any bodily injury, illness, death, medical treatment or property damage that may arise from, or in connection to, any services received from BBC. This liability waiver and release extends to BBC together with all owners, partners and employees.
